Questionnaires are used to help you determine if any potential contractors are compliant with workplace laws and regulations. If you don’t have a questionnaire, the following is a guide to help you with the process of setting one up.
1. Setting up Your Questionnaire

As this is a template, modifications for your specific requirements will need to be made. Instructions are provided at the beginning of the template. This is generally a straightforward process, whereby you will need to update some < insert tag > information, such as dates, e.g.:
Or, you can create your own digital questionnaire within the system. To locate this, head to Settings > Company > Pre-Qualification > Questionnaires.
Once in this section, click on the button to create a new questionnaire. From there, you can build the questionnaire.
A questionnaire can have five types of fields:
Heading configuration:
A heading is a read-only piece of text that can help divide an e-form into logical sections.
- Heading - the heading text that will be displayed on the form
Text configuration:
Text is a read-only part of the form, useful for instructions and explanations within the e-form.
- Content - the text that will be displayed on the form
Text field configuration:
A text field is a field type that a user will provide input for, for example entering a name or a contact phone number
- ‘Required’ - whether or not the user is forced to complete this field
- ‘Min chars’ - the minimum input length required from the user
- ‘Max chars’ - the maximum input allowed from the user
- ‘Numbers only’ - user input is limited to numbers
- ‘Letters only’ - user input is limited to letters
- ‘No restrictions’ - user input can be numbers or letters
Drop-down configuration:
A drop-down is a field type where a user makes a selection from predefined values
- ‘Required’ - whether or not the user is forced to complete this field
- 'Option' - the options within the drop-down
Multiselect configuration:
A multiselect is a field type that a user may choose one or more options for.

- 'Option' - the options within the multi-select
2. Uploading your Questionnaire to Altora
a) Navigate to Settings > Company > Pre-Qualification > Pre-Qualifications within your Altora admin dashboard.
b) Once in this section, you will have the option to create a new Pre-Qualification flow or edit an existing one. You see the function to either add a "Digital Questionnaire" or "Upload File". The Digital Questionnaire are the questionnaire forms that you can have created within the system.
- To edit an existing Pre-qualification click the "Edit" button next to the pre-qualification you would like to edit. Once in this section, ensure that you have selected Yes for "Do you want to add a questionnaire?" and then select which type of questionnaire you want to use.
- ii. To create a new flow click on the "+ New Pre-qualification" button in the top right corner to add additional Pre-Qualifications. Once in this section, ensure that you have selected Yes for "Do you want to add a questionnaire?" and then select which type of questionnaire you want to use.
c) Click "Continue" to proceed with creating your flow.
